Information for New Patients and Intake Forms

INITIAL VISIT

If you are a New Patient, we welcome you to Athens Integrative Medicine!

Please call us to set up your first appointment at (706) 850-2512

After your initial appointment, you will be able to easily book follow-up appointments online by clicking any of the Book Now buttons throughout the website.

Please bring any current (from the past year or two) relevant diagnostic blood work or other medical information with you, as well as any medications or nutritional supplements that you currently take. (Please bring the actual medication and supplement bottles.)

INSURANCE INFORMATION

Insurance does not cover Naturopathic Medical Care in the state of Georgia. Insurance may however cover Acupuncture treatments, depending on your policy. A Superbill can be provided to submit to your insurance company for reimbursement. We do not handle insurance billing in our office, and payment is due at the time of each visit. We accept all major credit cards. Flexible Spending Accounts and Health Savings Accounts often cover our Services.

INTAKE FORMS

After you call to book your New Patient appointment, you will be sent a link via email to fill out your intake paperwork online. Completing these intake forms online is our preferred method.

EXISTING PATIENTS

If you have already completed your initial appointment and are currently under Dr. Martha Allen’s care, you may easily book follow-up appointments by calling (706) 850-2512 or online by clicking here or on any of the Book Now buttons throughout the website.

If it has been over Five Years since you have been seen by Dr. Martha Allen, you will be considered a New Patient.
